From the press reports...looks like someone stole the computer(s)...

"TriWest spokesman Jim Kassebaum said computer equipment stolen from a
    company
office in Phoenix contained names, addresses, phone numbers, medical claim
histories and Social Security numbers for beneficiaries in a 16-state
    region.
Some credit card numbers also were in the files, officials said."

I'm betting the Fed's catch these guys. 500K+ records is too many to
    ignore.

Is this a case of "missing computer containing data" or "data known to be
    stolen from computer?"

Another fine example of the military HMO Tri-Care(aka Try Getting Care).

Data Stolen On 550,000 Tricare Beneficiaries In 16 States

Enrollment and claim files of 550,000 Tricare beneficiaries
across the 16-state Central Region of the military's managed
care network have been stolen, officials announced Dec. 23.
Missing are computer hard drives with names, addresses,
phone numbers, Social Security numbers, claims data and other
information on every beneficiary enrolled in the program
through TriWest Healthcare Alliance Corp., the managed care
support contractor for the Central Region.
